Family Caregiver Pay Map: Which States Pay Family Caregivers Through Medicaid

Every state runs Medicaid home care differently. This map answers, state by state, whether a spouse, an adult child or other relative, or a friend can be the paid caregiver, who employs them, and whether there is a waitlist. Of the 51 states verified so far, 20 do not pay spouses, 24 allow it in defined cases, and 5 allow it — and nearly all pay another family member.

The Home Care Agency is a free enrollment matching service — not a home care agency, not a Medicaid plan, and not a government agency. This research is educational. Eligibility, authorized hours and payment are always determined by the state program and, where applicable, the managed care plan.

How this map is verified

A state is published only after each verdict has been checked against the state's own materials — the approved 1915(c) Medicaid waiver application, the state administrative code, or the state Medicaid or aging agency's official pages — and the source is recorded on the state's page. We do not cite commercial websites. Each row shows the date its facts were last reviewed; rows are re-reviewed when a waiver renews or a rule changes, and at least every six months. Questions

Questions Families Ask About Paid Family Caregiving

Does Medicaid pay family members to be caregivers?
In most states, yes — through a Medicaid home care program that lets the person receiving care choose a relative or friend as the paid caregiver, either as an employee of a licensed agency or through a self-directed option. Which relatives qualify, and whether a spouse can be paid, is set by each state. This map shows the verified answer for each state.
Why does a state show "Under review"?
We publish a verdict only after checking it against the state's own materials — the approved Medicaid waiver application, the state administrative code, or the state Medicaid agency's official pages — and recording the source. States still being verified show "Under review" rather than an unsourced answer.
What does "In defined cases" mean for a spouse?
The state allows a spouse to be the paid caregiver only under conditions written into its rules, such as extraordinary care needs, no other qualified caregiver being available, or a specific program pathway. The state page lists the condition and the official source.
Does Medicare pay family caregivers?
No. Medicare does not pay family caregivers. These are Medicaid programs, so the person needing care must have, or qualify for, Medicaid in their state. Many people have both Medicare and Medicaid and can be served.
How do I use this to get started?
Find your state in the table and open its page. If we have partner home care agencies in your state, the page links to a short eligibility check and a free consultation. If we do not yet serve your state, the page points you to the program's administering agency. Eligibility is always determined by the state.
Can I cite or reuse this data?
Yes. The dataset is published under a Creative Commons Attribution 4.0 license. Cite it as "Family Caregiver Pay Map, The Home Care Agency" with the version date shown on this page, and link to this page. A CSV download is available on the methodology page.
