Who can you choose as caregiver?

  • Your Adult Child
  • A Sibling
  • A Relative
  • A Close Friend
  • A Neighbor

A paid caregiver must be at least 18, pass a criminal and abuse background check, and be able to perform the services in the service plan. By Iowa rule, the caregiver cannot be the member’s spouse, or the parent or step-parent of a member who is a minor. The caregiver may live in the same home as the person receiving care.

Questions

Frequently Asked Questions About Home Care in Iowa

What services does your home care agency provide?
We are an enrollment matching service — not a home care agency. We help Iowa families understand Medicaid’s Consumer-Directed Attendant Care service and match you with a Medicaid-approved provider agency in your area. The agency we match you with provides the actual caregiver services.
What is Consumer-Directed Attendant Care (CDAC) in Iowa?
CDAC is an Iowa Medicaid service, available under the state’s home- and community-based services (HCBS) waivers, that pays for hands-on help with daily activities at home. The member chooses who provides the care — in many cases a family member — and the caregiver works through a Medicaid-enrolled agency. Eligibility and covered services are determined by the program.
How do I apply for home care in Iowa?
If you live in Iowa, call us or fill out the form on this page. We'll help check your eligibility and match you with a Medicaid-approved provider agency that handles enrollment and caregiver setup.
Can I become a paid family caregiver for my parent or relative in Iowa?
In many cases, yes — an adult child, sibling, other relative, friend, or neighbor can be the paid caregiver, working through a Medicaid-enrolled agency. By Iowa rule, the member’s spouse (and the parent or step-parent of a minor member) cannot be the paid caregiver. Eligibility is determined by the program during enrollment — we help you check if you qualify and connect you with a Medicaid-approved agency that handles the rest.
Can a spouse be a paid caregiver in Iowa?
No — by Iowa rule, a CDAC provider cannot be the member’s spouse. Beyond the spouse (and the parent or step-parent of a minor member), you can choose almost anyone: an adult child, sibling, other relative, friend, or neighbor, subject to the program’s screening requirements.
How much do paid family caregivers make in Iowa?
Payment rates are set under the Iowa Medicaid program, not by us or any agency, and can change over time. The Medicaid-approved agency we match you with can walk you through current rates for your situation.
What happened to individual CDAC providers in Iowa?
Iowa ended the individual CDAC provider arrangement on December 31, 2025, after federal guidance on tax treatment. Care continues through two paths: agency-delivered CDAC, where the caregiver you choose works through a Medicaid-enrolled agency, and the Consumer Choices Option, where the member directs a budget as the employer. We match families with Medicaid-approved agencies for the agency path.
Does Medicare qualify me for Iowa CDAC?
Medicare alone does not qualify — CDAC is an Iowa Medicaid service, so the person needing care must have (or qualify for) Iowa Medicaid and an HCBS waiver. If you are not sure which coverage your loved one has, we can help you check.

Program Guide

Consumer-Directed Attendant Care (CDAC): What Iowa Families Should Know

What is Consumer-Directed Attendant Care?

Consumer-Directed Attendant Care (CDAC) is Iowa Medicaid’s service for hands-on help with daily living at home — bathing, dressing, meals, mobility — delivered by a caregiver the member chooses. It is available as a service under Iowa’s home- and community-based services (HCBS) waivers and is administered by the Iowa Department of Health and Human Services.

For families, the practical meaning is simple: if your loved one qualifies, Medicaid — not your family — pays for the care, and the person providing it can be someone your family already knows and trusts, working through a Medicaid-enrolled agency.

Who qualifies

The person needing care must have (or qualify for) Iowa Medicaid and be enrolled in one of Iowa’s HCBS waivers, which confirm through an assessment that the person needs the covered level of care. Attendant care must be included in the person’s service plan by their case manager. Medicare alone does not qualify.

The caregiver must be at least 18, pass a criminal and abuse background check, and be able to perform the services in the plan. By program rule, the member’s spouse — and the parent or step-parent of a member who is a minor — cannot be the paid caregiver.

Agency CDAC and the Consumer Choices Option

Since January 1, 2026, Iowa offers two paths. Under agency CDAC, the caregiver you choose is employed by a Medicaid-enrolled agency, which handles payroll, taxes, and program paperwork. Under the Consumer Choices Option (CCO), the member acts as the employer with a monthly budget, hiring their worker directly with the help of a financial management service.

Iowa previously allowed individuals to enroll as independent CDAC providers; that arrangement ended December 31, 2025 following federal guidance. Families who want a relative or friend to provide the care now do it through one of the two paths above. Our free service matches you with a Medicaid-approved agency for the agency path.

Choosing your own caregiver in Iowa

Iowa lets the member choose their caregiver: an adult child, sibling, other relative, friend, or neighbor can all serve, and the caregiver may live in the same home as the person receiving care. The two exclusions are the member’s spouse and, for a minor member, a parent or step-parent.

Eligibility for any caregiver is confirmed during enrollment — the agency and the program handle screening, training requirements, and payment.
