Under Structured Family Caregiving, the paid caregiver lives in the same home as the person receiving care and is supported and paid a per-day stipend through a Medicaid-approved SFC agency. A paid caregiver must be at least 18 and legally able to work in the U.S. By Ohio program rule, a spouse or legal guardian generally cannot be the paid caregiver. Eligibility for any caregiver is determined by the program during enrollment.

What is Structured Family Caregiving?

Structured Family Caregiving (SFC) is a service available under Ohio programs like PASSPORT. A caregiver who lives with the person receiving care — often an adult child or other relative — is supported and paid a per-day stipend through a Medicaid-approved SFC agency, which provides coaching, oversight, and support. Ohio added SFC to PASSPORT in late 2024, so many families have not yet heard of it.

Can I become a paid caregiver for my parent or relative in Ohio?

In many cases, yes — under Structured Family Caregiving, an adult child, sibling, grandchild, other relative, or close friend who lives with the person receiving care may be able to become their paid caregiver.
