Helping Ohio Families Navigate PASSPORT & Structured Family Caregiving
Ohio's Medicaid home care programs — including PASSPORT and its Structured Family Caregiving option — may allow a family member who lives with an eligible loved one to be compensated for the care they already provide. We help you check if you qualify and match you with a Medicaid-approved Ohio agency — free to families.
You are likely eligible if you have Medicaid. Fill out the form to see how fast you can get started.

Eligibility
Final eligibility is determined by the State of Ohio — we help you check in seconds.
Under Structured Family Caregiving, the paid caregiver lives in the same home as the person receiving care and is supported and paid a per-day stipend through a Medicaid-approved SFC agency. A paid caregiver must be at least 18 and legally able to work in the U.S. By Ohio program rule, a spouse or legal guardian generally cannot be the paid caregiver. Eligibility for any caregiver is determined by the program during enrollment.
