Program Guide
Community Choices is South Carolina's Medicaid waiver for home- and community-based long-term care, operated by the South Carolina Department of Health and Human Services (SCDHHS) through its Community Long Term Care (CLTC) division. It exists so that people who qualify for nursing-facility care can receive services at home instead — personal care, attendant care, companion services, home-delivered meals, and more.
For families, the practical meaning is simple: if your loved one qualifies, Medicaid — not your family — pays for approved in-home care, and the participant keeps the right to choose their provider.
The waiver serves seniors age 65 and older, and adults age 18–64 with physical disabilities, who have (or qualify for) South Carolina Healthy Connections Medicaid and meet nursing-facility level-of-care criteria. Medicare alone does not qualify.
Level of care is confirmed through a standardized CLTC assessment — the same criteria used for nursing-facility placement. Financial eligibility is determined by the Medicaid program, not by us and not by any home care agency. Waiver capacity can be limited, so there may be a waiting period for new enrollees.
Anyone — the individual or a family member — can make a CLTC referral by phone or online, and CLTC handles the assessment and service authorization from there.

The waiver includes a self-directed option for personal assistance: the participant (or a representative they choose) acts as the employer of record — selecting, scheduling, and, if needed, replacing their own worker — while a financial management service handles payroll and taxes.
