Eligibility

Who Is Eligible for Florida's Statewide Medicaid Managed Care Long-Term Care (SMMC LTC)?

Final eligibility is determined by Florida Medicaid — we help you check in seconds.

You're likely eligible if you…

  • Have or Qualify for Medicaid
Florida Medicaid long-term care has its own financial rules, determined by DCF. Medicare alone does not qualify.
  • Live in Florida
The person receiving care must be a Florida resident
  • Complete the ADRC Screening
New community enrollment starts with a phone screening that sets a priority score — most new applicants are placed on a priority list; some situations qualify for immediate enrollment
  • Need Help with Daily Activities
Bathing, dressing, mobility, meal prep, errands and more — the screening and assessment confirm the level of care
  • Enroll with a Long-Term Care Plan
Services are delivered through the LTC plan you pick; the Participant Direction Option lets you direct your own caregiver
Example Florida Florida Medicaid Medicaid card
You'll need an active Florida Medicaid (Florida Medicaid) card. Medicare alone does not qualify.

Who can you choose as caregiver?

  • Your Adult Child
  • A Sibling
  • A Relative
  • A Close Friend
  • In Some Cases, a Spouse

Under the Participant Direction Option, the person receiving care (or their representative) chooses and directs the caregiver. The caregiver must be at least 18 and pass a Level 2 background screening. In some cases a spouse can serve — the program applies extra conditions, and plan rules vary, so eligibility of any caregiver is confirmed during enrollment.

Questions

Frequently Asked Questions About Home Care in Florida

What services does your home care agency provide?
We are an enrollment matching service — not a home care agency. We help Florida families understand the Medicaid Long-Term Care program and match you with a Medicaid-approved provider agency in your area. The agency we match you with provides the actual caregiver services.
What is Florida's Medicaid Long-Term Care program?
Statewide Medicaid Managed Care Long-Term Care (SMMC LTC) is Florida Medicaid's program for ongoing help at home and in the community, run through managed care plans. Its Participant Direction Option (PDO) lets the person receiving care choose and direct their own caregiver for services like personal care, homemaker services, and companion care — in many cases a family member. Eligibility and covered services are determined by the program.
Is there a waiting list for Florida Medicaid home care?
For most new community applicants, yes. Enrollment starts with a phone screening through your local Aging and Disability Resource Center, which sets a priority score and rank — enrollment is offered as slots open, with the most fragile Floridians first. Some situations skip the list, like certain nursing-facility residents moving back home and Adult Protective Services referrals. If your loved one is already enrolled in the LTC program, there is no wait to choose or change providers — we can help either way, and we'll be straight with you about where your family stands.
Can I become a paid family caregiver for my parent or relative in Florida?
In many cases, yes — through the LTC program's Participant Direction Option, the person receiving care chooses their caregiver, and an adult child, sibling, other relative, or friend can serve. The caregiver must be 18 or older and pass a background screening. Eligibility is determined by the program during enrollment — we help you check where you stand and connect you with a Medicaid-approved agency.
Can a spouse be a paid caregiver in Florida?
In some cases. Florida's Participant Direction Option allows a spouse to serve as the paid caregiver under extra conditions — generally care beyond what a spouse would ordinarily provide, plus the standard worker requirements — and plan rules vary. The program makes the final call during enrollment; we can help you understand the path before you commit to anything.
How much do paid family caregivers make in Florida?
Payment rates are set under the Florida Medicaid program and the long-term care plans, not by us or any agency, and can change over time. The Medicaid-approved agency we match you with can walk you through current rates for your situation.
Does Medicare qualify me for Florida Medicaid home care?
Medicare alone does not qualify — the Long-Term Care program is Florida Medicaid, so the person needing care must meet its financial rules (determined by DCF) and its level-of-care rules (determined by DOEA). You do not need to be on Medicaid already to get screened for the priority list. If you are not sure which coverage your loved one has, we can help you check.
My loved one already has the Long-Term Care program. Can we switch to a caregiver we choose?
Often, yes. People already enrolled in the LTC program can use the Participant Direction Option and choose who provides their care — without going back through the waiting list. If you'd rather have a different agency or a family caregiver instead of a stranger, that is exactly the kind of change we help families make.

Program Guide

Statewide Medicaid Managed Care Long-Term Care (SMMC LTC): What Florida Families Should Know

What is Florida's Long-Term Care program?

Statewide Medicaid Managed Care Long-Term Care (SMMC LTC) is Florida Medicaid's program for ongoing care at home and in the community, administered by the Agency for Health Care Administration and delivered through managed long-term care plans. It covers hands-on help with daily living — bathing, dressing, meals, mobility — for Floridians who meet the program's financial and level-of-care rules.

For families, the practical meaning is simple: if your loved one qualifies and is enrolled, Medicaid — not your family — pays for the care, and through the Participant Direction Option the person receiving care can choose who provides it.

Who qualifies — and how the priority list works

Eligibility has two sides: financial rules, determined by the Department of Children and Families, and level-of-care rules, determined by the Department of Elder Affairs. Medicare alone does not qualify.

Most new community applicants start with a phone screening through their local Aging and Disability Resource Center. The screening produces a priority score and rank, and enrollment is offered as slots open — the most fragile applicants first. Some situations qualify for enrollment without the wait, including certain nursing-facility residents who have been in a Florida facility at least 60 consecutive days and want to move home, and adults referred through Adult Protective Services. We help families understand where they stand rather than guessing.

The Participant Direction Option — choosing your own caregiver

Inside the LTC program, the Participant Direction Option (PDO) lets the person receiving care act as the employer of their own caregiver for covered services such as personal care, homemaker services, and adult companion care. The caregiver can be someone the family already trusts — an adult child, sibling, other relative, or friend — and must be at least 18 and pass a Level 2 background screening.

A spouse can serve in some cases, under extra conditions — generally care beyond what a spouse would ordinarily provide — and plan rules vary, so spouse arrangements are confirmed case by case during enrollment.

Already enrolled? You can still choose

Floridians already enrolled in the LTC program do not go back on the waiting list to change how their care is delivered. They can choose the Participant Direction Option, choose a different provider agency, and choose who provides the care in their plan of care.

Our free service matches families — new applicants and current enrollees alike — with a Medicaid-approved agency, and is paid a flat marketing fee by the partner agency. Eligibility is always determined by the program.
